About
Hoi Yan Jasmine Chan
(Registered Social Worker, OCSWSSW, Canada)
Jasmine is a registered social worker providing counselling and psychotherapy. She is a Clinical Fellow of the American Association of Marriage and Family Therapy (AAMFT) and brings over 25 years of experience in counselling.
She began her career after graduating from the University of Hong Kong in 1999, working as a social worker with families, youth, and children from lower socioeconomic backgrounds through times of transition, struggle, and crisis. Over the years, Jasmine has supported thousands of clients. She spent 18 years working at a family service center in Hong Kong, where she also served as Assistant Director for three years, supervising social workers. She later relocated to Toronto to continue her psychotherapy practice.
Jasmine specializes in supporting clients experiencing depression, anxiety, trauma and relationship challenges, including couples,
parent-child, intergenerational, couple, and family dynamics. Her approach is compassionate and mindful and relational. She works primarily with an Emotionally Focused Therapy (EFT), trauma-informed and family systems perspective, and also offers Eye Movement Desensitization Reprocessing (EMDR) as a distinct, evidence-based treatment for trauma.
Since 2016, Jasmine has been teaching mindfulness after completing the Foundation Course in Teaching Mindfulness-Based Cognitive Therapy (MBCT) and is a Stage 5 Teacher with the Oxford Mindfulness (UK).
In addition to her clinical work, Jasmine provides supervision for psychotherapists and counsellors. As an Approved Supervisor Candidate working under the mentorship of an Approved Supervisor, she supports clinicians in their professional development, their pursuit of AAMFT clinical membership, and ongoing clinical growth.
In her own time, she enjoys family moment with her two teenage children. She enjoys hiking and cooking. She also finds hiking and cooking to be therapeutic activities that rejuvenate her inner being.
She is open to incorporate clients’ spirituality or faith into therapy as their request. If you are seeking personal growth, insight, reflection, relief from suffering, or a deeper reconnection with your inner self and loved ones, Jasmine is here to support you.

The counseling process
You are welcome to inquire about my services through a free 15-minute consultation session before scheduling a counselling session. This consultation can be booked through the booking page on my website.
After the consultation, we can proceed with the first counselling session. During this session, I will focus on understanding your current concerns, and together, we will set specific goals. The assessment process typically spans the first two sessions and involves exploring your past history and family of origin, identifying your strengths and vulnerabilities, and gaining a comprehensive understanding of your concerns. Based on this assessment, we will develop treatment goals, establish a direction, and create a plan tailored to your individual needs. During this time, we will also discuss the frequency of counselling sessions. As counselling progresses, goals and plans will be reviewed and adjusted as necessary to ensure they align with your evolving needs.
The therapeutic process begins with the assessment, which is an integral part of therapy itself. Many clients find relief in being listened to in a safe, trustful, and confidential environment.
In subsequent sessions, we will work collaboratively toward achieving the agreed-upon goals. This process will involve open discussions, reflections, and constructive feedback. Each session typically lasts one hour, with the option to extend by 15 or 30 minutes if needed.
