Hoi Yan Jasmine Chan
(Registered Social Worker, OCSWSSW, Canada)

Jasmine is a registered social worker, counselor, and Clinical Fellow of the American Association of Marriage and Family Therapy (AAMFT), with over 25 years of experience in counseling.

She began her career as a social worker after graduating from the University of Hong Kong in 1999, counseling families, youth, and children from lower socioeconomic backgrounds through times of transition, struggle, and crisis. Over the years, Jasmine has assisted thousands of clients. She spent 18 years working at a family service center in Hong Kong, where she also served as assistant director for three years, supervising social workers. She later relocated to Toronto to continue her work in psychotherapy.

Jasmine specializes in helping clients navigate depression, anxiety, trauma, self-image issues, gambling recovery, and complex relationship dynamics, including parent-child, intergenerational, couple, and family challenges. Her approach is compassionate and mindful, integrating family systems theory with emotionally-focused and cognitive techniques. She creates a safe, welcoming space for clients to experience healing, growth, and resilience.

Since 2016, Jasmine has been teaching mindfulness after completing the Foundation Course in Teaching Mindfulness-Based Cognitive Therapy (MBCT), developed by Professor Mark Williams of the Oxford Mindfulness Centre, one of her trainers. By combining mindfulness practices with cognitive behavioral strategies, she helps clients interrupt cycles of distress and reconnect with the present moment.

She has also completed AAMFT Fundamentals of Supervision course and provides supervision to counsellors, psychotherapist and social workers, supporting them in pursuing their careers, AAMFT clinical membership, and fostering professional development in the next generation.

In her own time, she enjoys family moment with her two teenage children. She enjoys hiking and cooking She also finds hiking and cooking to be therapeutic activities that rejuvenate her inner being.

The Counseling Process

You are welcome to inquire about my services through a free 15-minute consultation session before scheduling a counseling session. This consultation can be booked through the booking page on my website. Once you book the session, I will send you an intake and consent form as required by the regulatory college. You may complete and sign the form beforehand, or we can discuss it further during the consultation.

After the consultation, I will proceed with the first counseling session. During this session, I will review your intake and consent forms, focus on understanding your current concerns, and work with you to identify goals.

The therapeutic process begins with the assessment, which is an integral part of therapy itself. Many clients find relief in being listened to in a safe, trustful, and confidential environment. The assessment process typically takes place over the first two sessions. During this time, I will explore your past history and family background, identify your strengths and challenges, and gain a comprehensive understanding of your concerns. From this, I will collaborate with you to develop a personalized plan and direction for our work together. I will also discuss the frequency of counseling sessions with you during this phase. I will also discuss the frequency of counseling sessions with you during this phase.

In subsequent sessions, we will work collaboratively toward achieving the agreed-upon goals. As counseling progresses, I will review and adjust goals and plans as needed to ensure they continue to meet your evolving needs.
